FDS Review Report

How to Access FDS Review Report

- Go to Wealth Axis FP Tools > E2E Wizard
- Click on 11b Review Report
Review Report Navigation
Scenario Index

- Select an existing Scenario by clicking on a Scenario Name
- If you are creating a new scenario, click on the Create Scenario button. A pop up screen will appear as shown below.

- Enter the Scenario Name into the text box.
- Click OK. You will be taken to the Details page.
- Once completed click Next and move on to the next section
Notes

- Select the client for Advice
- Select the Advice Validity Period to allocate how long the advice valid for


- Select yes or no for Include Cover Page and Include Back Cover Page
- Select the Date to Use,
- if using a Custom Date, enter the Custom Date

- In the Select Report Contents, select the Documents that you wish to be included in the report by clicking the checkbox icon
- Select yes or no to Merge the Documents above
- Click on the checkbox to merge the following documents separately at the end of the wizard
Note: In this example we selected FDS/Opt-in

- Ongoing Service Agreement
- Once completed click Next and move on to the next section
Letter Editor

- Select the Area of Advice from the dropdown options, in this example we selected Administration Service
- Select Letter from the dropdown options, in this example we selected FDS/Opt-in
- Select the Period Ending date
- Enter in the Regarding (RE.) title and Content
- Select the Letter Closings from the dropdown options
- Select yes or no to include Print on Letterhead
- Once completed click Next and move on to the next section
Personal Details
In this section complete and check the Client(s):
- Details

- Tax and Resident Status

- Health Status

- Once completed click Next and move on to the next section
Service Packages

- Select the your Service Package in Category
Note: If you do not have a Service Package, learn how to setup a Service Package here
- Select the Client Value and Investor Type from the dropdown options
- Click on the checkbox to Include Renewal Notice (Opt-in)
- Enter the FDS Start Date and FDS End Date
Note: The dates has to be within a 12 month period
- Enter the Next Disclosure Statement Date

- Select the Source of Fees from the following

CommPay
One selected the FDS CommPay Fees page will open in the next page

IPS Feed
Once selected the Ongoing for IPS section will pop-up below, Edit or Delete any existing Ongoing Fees,

To add Fees, click the Add button and complete the information in the Ongoing for IPS section

Once completed click save

Invoice
Once selected the Invoice source will pop up and select between the following option in the dropdown menu:

- Invoice within Date Range
- Invoice Number

Manual

One selected the FDS Manual Fees page will open in the next page

Enter the required information in the Manual Fee section
Select the Offers & Services Source
- Click the checkbox if all Prescribe Service Offerings have been provided, if No state the reasons why
- Select the Service Offerings provided
- Select the Frequency of the various Client Communication options
- Select any Third Party Assistance
- List down Other Service Offerings
- Once completed click Next and move on to the next section
Opt-in and FDS Details
In this section confirm the Ongoing Fee Agreement, Signed and Renewal Date. In the Opt-in Details confirm that the Opt-in is required, the Next Opt-in Date and if it Subject to Grandfathering

In the Renewal section confirm the Renewal Notice Start and Response date

In the Disclosure Statement tab, confirm that a Disclosure Statement is required and the Next Disclosure Statement Date

In the Opt-in/FDS History section, you can view the previous FDS that have been provided

Once complete click Next to continue to complete the wizard
